WAIVER & RELEASE OF LIABILITY
I, the undersigned, acknowledge and agree to the following:

1. I voluntarily choose to participate in fitness activities, classes, and programs offered by Legacy Nutrition & Fitness, and I recognize that these activities may involve physical exertion that could result in injury or illness.

2. I accept full responsibility for any risks, injuries, or damages, known or unknown, that I might incur due to participation.

3. I hereby waive, release, and discharge Legacy Nutrition & Fitness, its owners, coaches, instructors, staff, and affiliated entities from any liability for injuries or damages arising out of my participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

4. I certify that I am at least 18 and mentally competent to sign this agreement.

Note: No children under the age of 18 are permitted in the facility or events unless they are enrolled in a youth program and have a completed waiver signed by a legal guardian.
